Register
To be on the team, you will need to do these things (each season):
Fill out the Interest & Communication Form if you haven't yet so that we can share what's going on.
Fill out the Player Info Form. Take a few minutes to fill this out as soon as you're pretty interested in being on the team so that we have your info. It does not commit you to be on the team. But you will need to fill it out eventually to join each season.
Fill out, sign, and return the Waiver & Release of Liability and Local Medical Authorization. You'll need a parent/guardian's signature (unless you're 18).
Digitally: You (or a parent/guardian) can fill it out and sign it digitally.
Download: You can also download a hard copy and bring it with you to your first practice. Or, filling it out and submitting pictures of it is acceptable.
At practice: You can also fill it out at the first practice (with a parent/guardian if under 18).
Pay Parkway Team Payments. Bring a check to practice or pay via Venmo (user id is timothy-dorsch, 314-518-7363, https://venmo.com/timothy-dorsch).
a) The team fee is $100 for new boys team players & all girls team players and includes the payment for your dark jersey top. Since our girls team is rebranding, they'll be getting new jersey tops. If you're a boys team player and interested in also getting the optional light jersey top or custom shorts, see below.
b) The team fee is $60 if you are a returning boys team player since you already have a jersey top. With the money you're "saving" as a returning player, consider getting the light jersey top and/or custom shorts (see below) if you don't have them yet.
c) If you're a boys team player and would like the $30 optional light jersey top and/or $40 optional custom uniform shorts, order them here: https://forms.gle/iaxfjwQw94EX2rvS6. The light jersey tops are for anyone, but will be required for Varsity A. We strongly encourage you to go ahead and order them if you think there's a good chance you'll be playing on Varsity A this year, next year, or in the Indy Invite. The shorts aren't cheap because, like the tops, they are individually printed full-color on fabric with your specific number, cut out, and then assembled.
By the first Sunday in March, pay the $40 Online USAU Fee and sign Online Waivers: Go to your USAU account at https://play.usaultimate.org/members (create an account if you haven't yet). Then:
a) Sign your online waivers on your USA Ultimate member's page and pay your $40 USA Ultimate "Youth Player" membership. See detailed instructions below. If you're a new player, make a note of your USAU ID #.
b) AFTER you have done the previous step, fill out this 30-second form so that we are notified and can add you to our official USAU team rosters.
(BONUS) If you've done all of these things, hug someone because you're all set!
Detailed instructions to navigate the USA Ultimate site, register, and pay the $40 USA Ultimate "Youth Player" membership:
—If you’re a new player:
Sign up for an account on USAUltimate.com (click the Join USA Ultimate button).
As you register, you’ll fill out the necessary waivers.
You’ll be assigned a membership #.
After you’ve finished registering, you’ll need to pay for your membership:
Click on Manage Account (upper-right of page). Click either the “Purchase Membership” or the “Pay My Dues” button.
Select “Youth Player” and proceed to payment.
—If you’re a returning player:
Log in to your account on USAUltimate.com.
Click on Manage Account (upper-right of page).
Click the buttons to sign your USAU waivers for the year.
Pay for your membership:
Click either the “Purchase Membership” or the “Pay My Dues” button.
Select “Youth Player” and proceed to payment.
Email your membership number to tim.dorsch@gmail.com. Then, we can add you to our team’s roster.
